The misunderstanding is the root of so many fun sexual fantasies.  In this case, I think it's the idea that a boy gets so excited when he hears what he wants to hear (that his girlfriend is completely on board with his being a sissy) that he doesn't even stop to think that maybe she's not talking about the same thing he's thinking about.
